You can print out an order form (Order Form), fill in all the information, and send along with payment to

Use the shopping cart method provided on this site. Enter the quantity of the items you wish to purchase and then click on the order button. When you "Check Out" you will be given a total cost, including shipping. Please print out this form (you may want to make two copies, one for your records) and mail the form, along with payment to the address above.
